Once a patient receives the AWP Healthcare device, an AWP Healthcare team member will reach out to assist with the initial setup. Below, supplemental information and instructional videos are provided to guide the use and setup of the devices for chronic care management.

Reminders & Confirmations of Data Readings

Each of AWP Healthcare's provided devices comes with a "gateway" transmitter, which allows for the AWP Healthcare team to receive data transmissions via cellular service.

Red Ring: A Reminder to Take Reading

Each morning, the gateway will display a red ring, reminding the patient to take their reading.

Green Ring: Reading Transmitted

After a reading has been taken, the gateway will turn green, indicating the reading was successfully sent to the HWP team.
